CNWL NHS FT Urgent Community Response service

Urgent Community Response (UCR) Teams are multidisciplinary clinicians who respond to a crisis in someone’s home (including residential and nursing homes), preventing an unnecessary hospital admission. 

They are a crucial service in Milton Keynes, both in helping people maintain their independence and manage complex and long-term conditions, and in easing the pressure on acute and emergency services. Patients can be referred to UCR Teams by a number of referrers, including 999, 111, Emergency Departments, care homes, GPs and primary care providers.

CNWL NHS FT Staying Steady - Falls prevention service

Staying Steady Milton Keynes offers specialist support to individuals, mainly aged over 65 years, who have fallen or are at risk of falling. 

We work with an individual and their carer to make decisions about the best way to reduce the risk of the person falling, raise awareness of risk factors for falling and make any necessary changes to their home to prevent falls.

CNWL NHS FT Community Mental Health Hub

Referrals to our hubs are made through healthcare professionals and social services. They can also be made through Milton Keynes Single Point of Access.

You can self-refer via 01908 724365, or your GP will refer you to us. In some cases, it may be more suitable for people to receive support from Talking Therapies instead of being referred to one of our hubs. You can self-refer to Talking Therapies, and they support people who feel anxious and worried or down and depressed.

CNWL NHS FT Neuro Conditions Service

The Neurological Conditions Service (NCS) consists of the Early Stroke Rehab Team (ESRT), the Neurological Rehabilitation Service (NRS) and the Neurological Clinical Specialist Team (NCST).

The teams work in partnership with people who have an acquired or long-term neurological condition and their families and/or carers.  They carry out specialist, person-centred assessments and work towards meaningful goals to facilitate independence and promote safety and function.  They provide specialist advice, support and education to enhance the quality of life and empower people to self-manage their condition.

The teams work alongside their colleagues in health and social care and voluntary services to make sure that care is maintained in the community setting.

CNWL NHS FT Community Nursing Service

The service is for patients aged 18 and over who are housebound or have a condition or procedure which can only be carried out in the home and have a nursing need. Patients must be registered with a Milton Keynes general practitioner (GP). Individuals can be referred by any healthcare professional (e.g. GP, specialist nurse, practice nurse, district nurse) or carer, providing they have given consent for the referral. Individuals can also self-refer.

CNWL NHS FT Home 1st Therapy

Home 1st Therapy provides assessment, care planning and therapy to rehabilitate and promote independence in patients’ own homes or as close to home as possible. It is one of a range of intermediate care services which provide therapies and rehabilitation in community settings for adults in Milton Keynes.

The team aims to assist patients to remain as independent as possible within their own home, supporting early discharge from hospital and avoiding hospital admission. Home 1st is a multi-disciplinary team of physiotherapists, occupational therapists, nurses, speech and language therapists, and rehabilitation assistants. They will work with patients and together agree on their personal goals to help achieve them within a carefully planned therapeutic programme.

This may last from a single visit to a maximum of six weeks, and is reviewed at regular intervals. The team accepts referrals from any health or social care professional, for example, GPs, district nurses, therapists, intermediate care team leaders, or any other community or hospital-based professional.

CNWL NHS FT Pulmonary Rehabilitation

The Milton Keynes Home1st Community Pulmonary Rehabilitation Service supports patients with a wide range of respiratory conditions, helping them manage their symptoms and improve their quality of life.

Pulmonary Rehabilitation is a clinically proven exercise and education programme to help improve exercise tolerance and breathlessness. This programme will reduce the risk of patients becoming unwell, possibly leading to hospital admissions and urgent GP appointments. Referrals accepted from Healthcare professionals.
